YNIA 2020 Trip to Detroit, Michigan

Yes, count me in for Young Neighbors 2020!

This form and a security deposit of $50.00 hold my place on the trip. I understand that there will be team meetings to prepare, as well as to discuss travel details, costs, and fund raising by the group.

All youth participants will be expected to participate in fundraising events.

Note: If Participant is under 18 years of age, the parent will need to sign a parental consent form.

Please send or drop off your $50 check at St. Matthew Parish Office, 130 St. Matthews Street, Green Bay, WI 54301 to complete your registration. Check should be made out to St. Matthew Parish, with YNIA 2020 in the memo portion.
